Inversions: I don't get it

cyrejWhatever hints were dropped towards unresolved murder mysteries, I didn't find them. Help?
Who killed Nolieti and Waren?
Not the Doctor, she was being threatened by them and has the means but is shown to only reluctantly give in to using violence and Culture means at the end for the first time. But on the other hand has used some of the ammunition on that dagger previously, I'm confused.
Adlain? Why kill two of the doctor's enemies, protecting her, while she is clearly in public somewhere else, only to orchestrate a third murder to frame and capture her for?
The King & network of spies and assassins (unrelated to Adlain)? Seems ignorant and uninvolved. Possible but I don't think what the author was getting at.
DeWar? Fits his characters but I think honestly shown to be ignorant of the Doctor's being there.
Just random assassinations and crimes? Are there just no hints for me to find?
Is it called Inversions because they have had a reversal of ideals and policies?
